Documents assessment, plan and interactions according to NVHC established guidelines and standards Maintains knowledge of community resources in order to provide for patient's case management needs Collaborates with Behavioral Health and Primary Care providers, and other members of the patient's interdisciplinary care team, in evaluating patient progress against the care plan; reviews and communicates with patient's revisions/updates of plan and/or goals. Serves as a resource with Primary Care Providers in conducting warm-hand offs to inform patients about the integrated treatment model and the benefits of behavioral health services and a referral to a Behavioral Health Provider. Assists Behavioral Health Director in monitoring behavioral health service delivery related to appropriate referrals for behavioral health services: scheduling initial and follow-up appointments as needed; assisting behavioral health and medical providers with referring patients to other services as needed. Assists Behavioral Health leadership in sustaining workflow pertaining to providers administering of behavioral health screenings such as PHQ2, PHQ9, etc. Serves as Behavioral Health community liaison with outside agencies; building community partnerships to share available services Required Qualifications Bachelor of Health and Human Services-related Degree from an accredited school of social work OR Current Nevada State Social Work Licensure (LSW) Preferred Qualifications Bilingual Spanish and English. Minimum one year work experience in an outpatient (or Inpatient) behavioral health setting. Skill in operating computer databases Ability to read, write, and calculate mathematical figures. Knowledge of ADA, CPT, and
ICD-9-CM
