Health Homes Case ManagerHarmonia Collaborative Care Community-based position requiring travel to client homes and community locations. Valid NYS driver's license and reliable transportation required. At Harmonia Collaborative Care, our Health Homes Case Managers help individuals navigate medical, behavioral health, and social service systems with consistency, compassion, and follow-through. - Provide Health Home care management and care coordination
- Conduct community and home visits
- Develop person-centered care plans
- Connect members with medical, behavioral health, housing, and community resources
- Coordinate with providers and support systems
- Complete timely documentation and maintain compliance standards What We're Looking For
- Bachelor's degree in human services or related field preferred
- Experience in case management, behavioral health, social services, or community support preferred
