This role provides clinical social work support within a behavioral health setting, including psychosocial assessment, mental health screening, disposition planning, and care coordination for patients with complex behavioral health needs. Responsibilities:
Conduct mental health screening interviews and psychosocial assessments for patients Document assessments, screening results, and clinical updates in the electronic medical record Develop disposition plans and provide appropriate mental health and social service referrals Participate in multidisciplinary care coordination with mental health and medical providers Provide crisis intervention, benefits counseling, and case presentation support as needed
Qualifications:
At least 1 year of experience in a mental health clinic or hospital setting Master's degree in Social Work Active New York State Licensed Master Social Worker (LMSW) license Experience with psychosocial assessments, mental health screening, and risk assessment
Desired Skills:
Experience with PTSD or trauma-informed care Experience supporting patients with persistent mental and medical health conditions Experience using Epic Strong organizational skills and experience with disposition planning or case presentation
