Flexible scheduling for many roles Position Summary Assist clients in attending scheduled physical health and wellness appointments and support follow-through with treatment recommendations. Monitor and document client compliance with medical and wellness plans. Support clients in transitioning to appropriate community-based health and social service providers. Provide safe transportation for clients using personal vehicle in accordance with agency policies. Maintain accurate, timely documentation and data entry in required systems meeting agency defined standards and expectations. Collaborate with healthcare providers and community partners to coordinate client care. Educate clients on health management, appointment preparation, and self-advocacy. Observe and report changes in client health, functioning, or risk concerns. Maintain professional boundaries while building supportive, therapeutic relationships. Perform services in various environments including hospitals, jails, clinics, the community "meeting the client where they are." Perform other duties as assigned. What We Value in our next Transitional Case Manager Associate degree or Bachelor's degree in administrative, human services, or related field.
