Gracehaven is seeking a compassionate and dedicated Community-Based Case Manager & Clinician to join our team. You will provide trauma-informed clinical services, case management, and support to youth, families, and the broader treatment team.
Responsibilities:
Individual and family therapy Crisis intervention and safety planning Comprehensive assessments and treatment planning Collaboration with families, community partners, and other providers Client advocacy and discharge planning Maintaining accurate documentation and meeting quality assurance standards
Qualifications:
Master's degree in Social Work/Counseling or a related field LSW or LPC license in Ohio (or eligible) Experience working with trauma, especially sexual violence and trafficking Excellent clinical and communication skills Agreement with the Central Ohio Youth for Christ and Gracehaven Standards of Conduct and Statement of Faith.
