We provide individualized, whole-person care and are committed to improving the lives of our patients, families, and community. We are seeking a Caseworker to support adult clients in mental health and substance use treatment programs.

Key Responsibilities:

Provide case management for clients in IOP/Day Treatment Support treatment planning, goal setting, and life skills development Facilitate mental health and substance use groups Connect clients to community resources and address barriers to care Collaborate with clinical and medical teams Complete timely and accurate documentation Assist with transportation and community-based services

Requirements:

Active CDCA required 3+ years of case management experience 2+ years in mental health and/or substance use Experience with QMHS documentation and EHR systems Previous Ohio Medicaid provider and active NPI required Strong communication, organization, and engagement skills Ability to work with diverse populations and in community settings
