Case Manager - Qualified Mental Health Professional Changing Lives Toledo, OH Job Details Full-time $45,000 - $65,000 a year 1 hour ago Benefits Paid time off Qualifications Individual therapy HIPAA Records management Bachelor's degree Direct support Passenger van operation Full Job Description Job Overview Work hours are 12-8pm must be 25 or older and be able to drive the work vans Three years experience in mental health and/or a bachelors degree Is required LSW/LPC preferred Duties Provide individual and group sessions. Maintain accurate medical documentation and records in compliance with HIPAA regulations. Collaborate with interdisciplinary teams to ensure holistic patient care and effective case management. Provide crisis management support as needed, ensuring client safety and well-being. Skills The successful candidate will have experience or proficiency in the following areas: Crisis Intervention strategies and Crisis Management skills. Ability to work effectively with diverse populations including individuals with autism and developmental disabilities. Proficient in Group facilitation and Direct Support roles.
