This is more than a traditional case management position. In addition to providing case management services, you'll have opportunities to develop treatment plans, conduct telehealth appointments, facilitate psychoeducational groups, and receive training in supervised visitation services. This is primarily an office-based position with telehealth responsibilities. Limited community work may be required; routine transportation of clients is not part of this position. The ideal candidate:
Understands the impact of addiction, mental illness, trauma, and social determinants of health. Maintains healthy professional boundaries while providing compassionate, person-centered care. Is technologically proficient and comfortable using electronic health records, Zoom, Microsoft Office, and other digital tools. Learns new systems quickly and embraces change. Produces timely, accurate, and high-quality documentation. Is dependable, organized, and accountable. Is a self-starter who takes ownership of their work and manages their responsibilities independently. Communicates professionally with clients, referral sources, courts, healthcare providers, and community partners. Thrives in a collaborative team environment while working independently. Essential Responsibilities Complete comprehensive case management assessments. Develop and maintain individualized case management service plans. Develop, review, and update individualized treatment plans. Coordinate referrals and connect clients with community resources including housing, employment, education, healthcare, transportation resources, and recovery supports. Assist clients in overcoming barriers to treatment and recovery. Conduct telehealth appointments utilizing Zoom. Facilitate psychoeducational and recovery groups as needed. Receive training to provide supervised visitation services. Collaborate with counselors, medical providers, courts, probation officers, child welfare agencies, referral sources, and community partners. Complete timely, accurate, Medicaid-compliant documentation within the agency's electronic health record. Participate in multidisciplinary treatment team meetings. Maintain a productive caseload that consistently meets agency productivity expectations. Independently manage their schedule by proactively scheduling, confirming, and following up with clients. Maintain a full schedule by minimizing cancellations and promptly rescheduling missed appointments. Complete documentation, treatment plans, and case management service plans accurately and within established timeframes. Utilize technology efficiently to support quality client care. Identify barriers to client success and proactively seek solutions. Adapt to organizational growth, new technology, and evolving clinical practices. Demonstrate professionalism, dependability, integrity, and continuous improvement. Minimum Qualifications Candidates must meet one of the following: Bachelor's degree in a behavioral health or related field; or Associate's degree in a behavioral health or related field; or High school diploma or GED with a minimum of three (3) years of qualifying behavioral health or social service experience sufficient to meet Ohio Qualified Behavioral Health Specialist (QBHS) eligibility requirements. Additionally, candidates must: Be eligible to obtain Qualified Behavioral Health Specialist (QBHS) designation. Current Ohio CDCA certification preferred. Candidates who are willing and eligible to obtain a CDCA will also be considered. Possess a valid Ohio driver's license. Successfully verify all required education, certifications, and licensure prior to employment. Preferred Qualifications Behavioral health or substance use case management experience. Treatment planning experience. Experience facilitating psychoeducational or recovery groups. Experience providing telehealth services. Knowledge of Ohio Medicaid documentation requirements. Experience using electronic health records. Experience working with courts, probation departments, child welfare agencies, or community partners. Ready to Make a Difference?
