Job Description

Overview HOPE Community Services, Inc. is a leading outpatient treatment provider in south Oklahoma City. We provide compassionate trauma-informed care to individuals whose lives have been impacted by mental health and substance use disorders. We do this by providing compassionate and evidence-based treatment, aimed at improving the quality of life for those we serve. We are currently seeking a dedicated and client-focused Engagement/Suicide Pathway Case Manager to join our team. The ideal candidate will have experience in case management, crisis response, and community-based services. About the Position As an Engagement/Suicide Pathway Case Manager, you will provide advocacy, linkage, case management, and independent living skills training to individuals experiencing significant mental health challenges. You will support clients through transitions in care, including post-hospitalization, and ensure timely engagement in services, particularly for those referred through the Suicide Pathway. This role requires consistent, face-to-face contact in community, home, and clinic settings, as well as strong documentation practices and collaboration with internal teams. About the Program The Engagement team provides intensive, community-based support to individuals who require frequent contact and timely intervention. The program focuses on rapid engagement, continuity of care, and a recovery-oriented approach that promotes safety, stability, and independence. Services are designed to meet individuals where they are and provide meaningful support during critical points in their recovery journey. Requirements Bachelor's degree in psychology, social work, or related mental health field. Must meet ODMHSAS and HOPE requirements for Behavioral Health Case Manager II certification. Valid Oklahoma driver's license and compliance with HOPE insurance requirements. Case management and crisis intervention experience in behavioral health or social services preferred. Skills Ability to assess client needs, organize services, and coordinate resources. Strong interpersonal and engagement skills, with the ability to build rapport and maintain consistent client contact. Proficiency in clinical documentation and adherence to confidentiality guidelines. Ability to apply trauma-informed principles and provide services in diverse community settings. Comfortable using electronic health records and other technology platforms. Strong organizational, analytical, and problem-solving skills. Experience in crisis response, care transitions, and working with individuals with complex behavioral health needs preferred.
