COMPENSATION
Hourly Pay Range:
$30.303862•$38.184620 Salary offers will be made within the posted pay range and will be based on candidate's experience (paid or unpaid) that is directly relevant to the position.
JOB DETAILS AND QUALIFICATIONS
Clackamas County's Social Services Division is seeking Case Managers to join the Developmental Disabilities Services Program. Case Managers working in this program manage community-based services by providing case management, crisis intervention, and residential services to Clackamas County children and adults with intellectual and developmental disabilities. Case managers in these positions are responsible for authorizing and monitoring Medicaid payment for services. Required Minimum Qualifications/Transferrable Skills:
• Must meet the minimum standards for a Service Coordinator/Case Manager under Community Developmental Disability Programs as defined in the Oregon Administrative Rule (OAR) 411-320-0030 and 411-415-0040: Three (3) years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; OR An associate's degree in a behavioral science, social science, or a closely related field and two (2) years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; OR A bachelor's degree in any field and one (1) year of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; OR A bachelor's degree in behavioral science, social science, or a closely related field At least 1 year of current (within past 4 years) experience with Intellectual and Developmental Disabilities services At least 1 year of experience providing case management services such as developing individual service plans, safety plans, skill training plans and conducting client assessments At least 1 year of experience conducting comprehensive assessments of client needs as a regular part of a job, independently and usually without review by a supervisor, manager or senior employee At least 1 year of experience monitoring client progress and ensuring service plans are carried out. (This may involve visiting clients in homes, hospitals and/or residential care facilities) Experience in collaborating with community agencies, organizations or law enforcement Thorough knowledge of federal and state regulations that define the requirements and responsibilities for case management and corresponding services in order to assist the developmentally disabled populations

TYPICAL TASKS
Case Management, Assessment & Service Planning Conduct home visits, and person-centered service planning for individuals receiving Waiver, K-Plan, and developmental disability services in compliance with Oregon Administrative Rules (OAR) and Oregon Office of Developmental Disabilities Services (ODDS) policy. Develop, authorize, coordinate, and monitor Individualized Service Plans (ISP), including annual reviews, transition planning, entry/exit meetings, and interdisciplinary service coordination. Coordinate medical, behavioral, residential, employment, transportation, and community-based supports while promoting equitable, culturally responsive access to services. Documentation, Compliance & Fiscal Oversight Maintain accurate case documentation, assessments, authorizations, and service records in ODDS-approved databases and county systems in accordance with OAR, Medicaid, and ODDS requirements. Authorize and monitor Medicaid-funded goods and services, including use of eXPRS/Plan of Care systems, prescribed payment methodologies, and expenditure guidelines. Monitor provider compliance, service implementation, incident reporting, and adherence to OAR and ODDS requirements, including review of service payments and monitoring checklists. Maintain working knowledge of laws, regulations, eligibility criteria, reporting requirements, and policy changes affecting individuals receiving developmental disability services. Coordination, Advocacy & Systems Navigation Collaborate with families, schools, Child Welfare, healthcare providers, Oregon Vocational Rehabilitation Services (OVRS), service providers, and community partners to coordinate supports and transitions across systems. Advocate for individuals and families by providing referrals, resource navigation, rights education, and support accessing entitlement, financial, medical, legal, employment, and community-based services. Coordinate youth-to-adult transitions, Employment First activities, and cross-system planning to support long-term independence and service continuity. Participate in guardianship, healthcare representative, and alternative resource planning processes in accordance with OAR and person-centered planning practices. Crisis Response, Monitoring & Professional Practice Assess, coordinate, and respond to crisis situations, emergencies, psychiatric or behavioral concerns, protective service needs, and after-hours response as appropriate. Receive, report, and coordinate responses to allegations of abuse or neglect in compliance with state reporting systems, Child Welfare, and protective services requirements. Monitor in-home and waiver services, adjust service plans as needed, and facilitate referrals for nursing, residential, relief care, and specialized supports. Participate in trainings, committees, program meetings, and ongoing professional development while supporting county values of equity, accountability, and service excellence.
WORK SCHEDULE
This position is included in the County's alternate workweek program, working 37.5 hours during a standard workweek of Monday through Thursday (Fridays off). Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended. This position is eligible for hybrid on-site/telework and based on the Department's business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
