Job Description

Blended Case Management Supervisor Position Description:
We are looking for a Blended Case Management Supervisor to join our Team! Nulton Diagnostic & Treatment Center is accepting applications for a Blended Case Management Supervisor for our Cambria County teams. The successful candidate will provide Blended Case Management Services and Supervision in accordance with Federal, State, and Agency requirements.
Education, Experience, Skills, Training:
The following minimum requirements shall be met by supervisors of blended case management services: A. A master's degree in social work, psychology, rehabilitation, activity therapies, counseling or education and 3 years mental health direct care experience; or B. A bachelor's degree in sociology, social work, psychology, gerontology, anthropology, history, criminal justice, theology, counseling, education, or be a registered nurse, and 5 years mental health direct care experience, 2 of which shall include supervisory experience; or C. A bachelor's degree in nursing and 3 years mental health direct care experience. Starting Salary $45,000-$48,000 Annual Bonus of up to $10,000. The successful applicant must have a valid Driver's License and be able to obtain Act 33, Act 34 and FBI clearances. This is a full-time, salaried position which includes training, competitive salary, and medical, dental and vision benefits, as well as, paid vacation and paid holidays.
