At Merakey, we put our heart and soul into everything we do. We seek a Case Manager to join our Behavioral Health Urgent Care (BHUCC) team in Philadelphia, Pa. This posting is Full-time, First Shift (Monday - Friday 10:30am - 7:00pm EST). Earn $26.34/hr.! We are a Behavioral Health Urgent Care/Walk-In Center for adults needing non-crisis same-day care in Philadelphia. Consider it "urgent care" for individuals experiencing emotional or behavioral health concerns. Individuals will participate in resolution-focused services and aftercare planning to prepare for long-term recovery and connection to ongoing care. We serve individuals regardless of income or insurance status. Our services do not replace the need for ongoing mental health care; rather, we aim to reduce avoidable visits to crisis centers and emergency rooms by helping people get the mental health care they need when they need it. Position Details The Case Manager provides immediate and follow-up case management services to individuals experiencing urgent behavioral health challenges. The primary responsibility will be to offer comprehensive support, including benefit enrollment, information about external resources, addressing the Social Determinants of Health, referrals, and completing outreach contact calls. The position will require a collaborative and empathetic approach to help individuals access the resources necessary to address their health needs and improve overall well-being. Requirements:
The ideal candidate will possess the following qualifications: Bachelor's degree in Social Work, Psychology, Counseling, or a related field. Previous experience in case management, social work, or related roles, preferably in behavioral health or crisis settings. Knowledge of public and private benefit programs and community resources related to Social Determinants of Health (SDOH). Excellent communication and interpersonal skills to engage effectively with individuals and colleagues. Empathy, compassion, and a person-centered approach. Ability to work collaboratively in a multidisciplinary team environment. Strong organizational and time management skills to manage case management responsibilities effectively. Familiarity with electronic health records (EHR) systems and healthcare technology. Bilingual is a plus.
