Job Description
The Forensic Case Manager is a behavioral health professional working with individuals in the criminal justice system. They determine client eligibility and assist clients in identifying and linking with needed resources through assessment and completion of screenings. Work is conducted on-site at prisons, other community sites, and in the office via telephone.
Duties and Responsibilities:
Complete diagnostic assessments and screenings with assigned clients. Educate individuals in the criminal justice system regarding access to needed services. Assist contacts at provider agencies and within the criminal justice system by sharing information about service access procedures and resources. Assure coordination of services and collaboration with probation for individual clients from Erie County Prison and State Correctional Institutions, as necessary. Make essential referrals and coordinate the sharing of critical client information. Facilitate inter-agency team meetings on behalf of clients for the purposes of care planning and service coordination. Complete all documentation according to established procedures and maintain appropriate client records. Collect and document required data. Maintain proficiency in the use of specified data bases.
Qualifications:
Must have a Master's degree in Counseling, Social Work, Psychology, or related field and at least two year's experience in the behavioral health field. CAC, CAAP, or other addictions certification preferred. Professional licensure preferred. Act 33/34 Clearances required.
