We are currently seeking a dedicated and compassionate Targeted Case Manager to join our dynamic team and make a positive impact on the lives of our clients. Job Description As a Targeted Case Manager at Pickett Fences Family Stabilization and Mental Health Services of South Florida Inc you will play a vital role in the coordination and delivery of services to individuals who require specialized assistance in navigating the complex healthcare and social services systems. Your primary responsibility will be to assess the needs of our clients, develop individualized care plans, and connect them with the appropriate resources and support to improve their overall well-being.
Key Responsibilities Client Assessment:
Conduct thorough assessments of clients' physical, mental, emotional, and social needs to determine their eligibility for targeted case management services.
Individualized Care Planning:
Develop and implement individualized care plans in collaboration with clients, focusing on their goals, strengths, and needs.
Resource Coordination:
Connect clients with essential healthcare, social, and community resources, ensuring they receive the appropriate services and support.
Advocacy:
Serve as an advocate for clients by representing their interests and ensuring their voice is heard within the healthcare and social services systems.
Monitoring and Follow-Up:
Continuously assess the progress of clients, conduct regular follow-up meetings, and make necessary adjustments to care plans to ensure the best outcomes.
Documentation:
Maintain accurate and up-to-date client records, including assessments, care plans, and progress notes, in compliance with regulatory requirements.
Collaborative Teamwork:
Collaborate with a multidisciplinary team of healthcare professionals, social workers, and community organizations to enhance client care.
Education and Support:
Educate clients and their families about available resources, services, and self-advocacy, empowering them to manage their health and well-being. Qualifications Bachelor's degree in Social Work, Psychology, Counseling, Nursing, or a related field (Master's degree preferred). State licensure or certification as required by Florida Certification Board and the Agency for Healthcare Administration. Previous experience in case management or related healthcare/social service roles. Strong interpersonal and communication skills. Knowledge of community resources and social services. Ability to work independently and as part of a team. Empathy, compassion, and a commitment to client-centered care.
