We are actively seeking motivated Certified Behavioral Health Case Manager contractors to join our ambitious team for continued growth.
Contractor Position Available :
Behavioral Health Case Manager ( CBHCM )
OBJECTIVE
To identify and assess client and family needs and evaluate, coordinate and ensure necessary services and/or treatment are provided; provide in-home services to clients; complete required assessments; assist and counsel individuals and families by using such activities as delineating alternatives, helping to articulate goals and providing needed information. Everything must be done per Medicaid guidelines.
ESSENTIAL FUNCTIONS MAY INCLUDE
Provide TCM Services (information, plan referral and linkages) as needed Develop and maintain clinical records and program documentation according to contract (assessments, treatment plans, progress notes, etc.) per Medicaid guidelines Coordinate and advocate for services with other professionals and para-professionals Conduct community outreach as needed Arrange for transportation of clients to appointments, community agencies, etc., as needed Work with the client's family to address issues related to the client's service plan per Medicaid guidelines Monitor the appropriateness of services to the client per Medicaid guidelines Complete billing related documentation for the Medicaid billing department ensuring billing documentation is accurate and complete per Medicaid guidelines Participate in clinical review of client for services and enhanced placement when appropriate per Medicaid guidelines Attend staffing's on behalf of the client to ensure the needs of the client are being addressed fully (schools, clinical, etc.) Complete and submit documentation related to authorization for Mental Health Targeted Case Manager (TCM) services per Medicaid guidelines Participate in the quality improvement process. per Medicaid guidelines Submit all documentation and billing logs per Medicaid guidelines
JOB REQUIREMENTS & QUALIFICATIONS

EXCEPTIONS APPLICANTS MUST OWN A LAPTOP, PRINTER , ACTIVE CELL PHONE , VEHICLE AND ACTIVE CAR INSURANCE.
Bachelor's degree or greater from an accredited university or college with a major in a Human Services related field and a minimum of two year full time or equivalent experience working with severely emotionally disturbed children and adults.- or -Bachelor's Degree in a non Human Services related field from an accredited university or college and three years full time or equivalent experience working with severely emotionally disturbed children and adults. Applicant MUST have the Florida Board Certification (CBHCM) for this position .

: Ability to Speak Spanish and English Fluent Excellent assessment skills and intervention strategies Ability to identify and understand environmental stressors Ability to observe and report accurately on the functioning of individuals & families Ability to handle confidential information appropriately Strong verbal and written communication skills Awareness of sociocultural characteristics of service population Ability to make home visits Ability to set appropriate limits and boundaries with clients Knowledge of community resources Ability to work flexible hours as required Ability to act decisively when necessary to protect client and oneself
