About the Role As an Integrated Behavioral Health Clinician , you will provide direct clinical services to patients of all ages within a collaborative, team-based care model. This includes supporting individuals and families affected by substance use disorders and contributing to program development and quality improvement initiatives. Key Responsibilities Integrate behavioral health into primary care services through a team-based approach Provide individual, family, and group counseling for mental health and substance use concerns Conduct diagnostic evaluations and develop patient-centered treatment plans Deliver care via in-person and telehealth services Provide crisis intervention and urgent behavioral health support Support health behavior change and wellness in treatment planning Facilitate chemical dependency orientation sessions Educate patients and families on addiction, recovery, and available resources Coordinate care across clinical teams and community-based organizations Participate in grant-funded program implementation and reporting Contribute to program development, evaluation, and quality improvement efforts Maintain accurate and timely clinical documentation Support training and mentorship of students and trainees Participate in outreach, prevention, and community education initiatives What We're Looking For Required:
Master's or Doctoral degree in Social Work, Marriage & Family Therapy, or related field from an accredited program License-eligible in the State of Hawai'i (LCSW or LMFT) Minimum of 3 years of relevant experience Preferred:
