Adult Mental Health (MH) Housing Housing is an essential component of recovery and independence. View Point Health offers a full continuum of residential services to adults with psychiatric needs and provides stability for individuals experiencing homelessness. The Adult MH Supportive Housing Program is in search of a passionate, skilled, and dedicated Clinician to become a part of our dynamic team! Under general supervision, the Residential Support Clinician provides clinical assistance to all aspects of the Supportive Housing Program in Gwinnett, Rockdale and Newton counties to include Intensive Residential Treatment and Group Homes, Semi-Independent and Independent Level Placements, as well as any Supportive Housing expansion programs. Under supervision, learn to perform psychosocial assessments, treatment planning/monitoring and discharge planning. Provides counseling/therapy to clients requiring complex intervention. Performs commitment determinations and/or executes orders of involuntary commitments.
Duties & Responsibilities:
Completes enrollment and related processes for individuals seeking supportive housing services throughout the Gwinnett/Rockdale/Newton areas. Monitors and updates program rosters to complete and/or ensure timely completion of client authorizations, mental health clinical assessments and other required program documents; monitors and resolves related failed claims. Assists with program referral and placement activities to include participation in client screenings, communicating with outside agencies to address client needs, providing recommendations for the appropriate level of residential care and collection of required eligibility documentation. Participate as a member of the treatment team for residential clients to include but not limited to participation in required treatment team meetings, updating client residential treatment plans as required by State and agency guidelines, and participating in client discharge planning and processes. Provides limited psychoeducation and psychosocial support needed to assist residential clients in coping with chronic, acute, or terminal illnesses, mental, physical and/or developmental disabilities, assists with crisis intervention services and execution of involuntary commitment orders. Provides limited clinical assistance and education to support individuals in restoring and developing skills for living independently. Serves as a clinical resource to residential program staff for administrative information and technical assistance; assists with site audit preparations; promotes Housing First principles. Assists with administrative or clinical meetings; completes required documentation (clinical notes, assessments, treatment plans) for residential billing in an accurate and timely manner. Participates in clinical supervision, staff meetings and training to increase competence and professional development as outlined and required by the agency and Program Director Receives, reviews, and processes client referrals from Piedmont Rockdale Hospital to meet CCBHC standards Completes clinical assessments and/or refer clients to the Rockdale Center for additional follow-up Monitors and tracks referrals for CCBHC using assigned spreadsheet and submit monthly Performs other related tasks as assigned by the Director or Program Manager Other program related duties.
Minimum Qualifications:
Master's degree in social work and counseling from an accredited college or university and eligibility to be licensed and two (2) years of experience in the area of assignment or two (2) years of experience at the lower-level Social Worker, Licensed 1 (HCP050) or position equivalent.
Note:
Some position s require licensure in the state of Georgia as an LAPC or LMSW.
Note :
Must be 21 years of age and must have valid Georgia Driver's License and a clean motor vehicle report.
Preferred Qualifications:
At least One (1) year of experience working with MH/IDD population. LPC or LCSW license preferred, in the state of GA. Experience working in a residential program.
Requirements/Competencies:
Must be able to lift 20 pounds. Requires long periods of sitting/standing. Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy. Candidates for selection must pass a criminal background check (including fingerprinting). Must pass a pre-employment drug screen and subject to random drug screens.
