Job Description

Summary Provides behavioral health consultation support, care coordination, resource navigation, and outreach activities for the Child Psychiatry Access Network (CPAN) and PeriPAN programs. Serves as a primary point of contact for participating providers, facilitates access to behavioral health resources and referrals, supports psychiatric consultation workflows, and promotes program engagement across Texas. This position is located at our McNair Campus and allows for an opportunity to work on site and telecommute. Job Duties Serves as the first point of contact for the CPAN/PeriPAN call line, assessing caller needs within scope of licensure, obtaining provider intake information, and completing required documentation and call forms in designated systems and tracking platforms. Provides curbside consultations to the primary care team. Routes psychiatric consultation requests to CPAN/PeriPAN psychiatrists as appropriate and facilitates communication between providers and consulting psychiatrists. Provides crisis intervention information, supports development of safety and clinic crisis protocols, and validates referral and resource information. Manages eConsult requests received through provider portals and ensures timely coordination and follow-up. Returns emails, text messages, and phone calls received after business hours to support continuity of care and provider engagement. Manages program communication platforms, including texting services, and provides timely support to participating providers. Serves as a liaison among providers, patients, families, community agencies, and healthcare professionals to facilitate access to behavioral health services and resources. Assists providers in identifying and accessing behavioral health referrals based on patient needs, insurance coverage, and geographic location. Researches, verifies, and maintains accurate behavioral health referral information and community resources. Contacts referral agencies and community partners to obtain updated information and maintains referral databases, including the Welnity resource database. Ensures resource information shared with providers is current, accurate, and timely. Assists with CPAN/PeriPAN presentations, CME presentations, and Project ECHO presentations as needed. Participates in CPAN/PeriPAN workgroups. Conducts and supports team outreach and engagement activities via text, phone, email, or clinic visits as needed. Performs other duties as assigned. Minimum Qualifications Master's degree in a related field. No experience required. Licensed Master Social Worker (LMSW), Licensed Professional Counselor-I (LPC - I), Licensed Psychological Associate-I (LPA-I), Licensed Marriage & Family Therapist-A (LMFT-A), Licensed Marriage & Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW). Preferred Qualifications Perinatal experience.
