Position Requirements:

Registered Nurse (RN) Licensure, Licensed Social Worker (LSW) licensure, Licensed Practical Nurse (LPN), Bachelors degree in Psychology, Gerontology, Health Care Administration or Sociology.

Details:

Case Managers will have an interactive contact with, primarily, older adult clients by phone and in-person visits Assist clients in gaining access to any necessary services regardless of funding source which enable clients to remain in their community living arrangement. Maintain ongoing contact with service providers to monitor service delivery and resolve service provision problems. Maintains contact with informal caregivers to obtain necessary information and coordinate formal and informal services. Coordinating and monitoring delivery of long-term care services Case Managers will be traveling about 50% of the time and completing visits within 2 counties of the surrounding 9 counties (Ashland, Crawford, Huron, Knox, Marion, Morrow, Richland, Seneca and Wyandot) with mileage reimbursement
