Job Description
Overview Outreach case management serving adults with mental illness in their homes and the community. Services are designed to assist individuals in achieving psychiatric stability, functional improvement, and successful community living through assessment, service planning, care coordination, and skill‑building interventions consistent with medical necessity and recovery‑oriented principles. Activities include contributing to assessments and recovery plans as part of a multi-disciplinary team and direct assistance with household management and teaching independent living skills. Staff also provide supportive counseling, illness management and recovery assistance and psychoeducation and acts as a liaison with medical providers and external institutions in order to connect people with benefits and entitlements.
Essential Duties & Responsibilities:
Provide ongoing face-to-face, community based mental health case management, including service coordination, client advocacy, and skills‑based support Coordinate care with psychiatrists, therapists, hospitals, partial care programs, and other behavioral health providers Support clients in medication management adherence by coordinating with prescribing providers and implementing medication maintenance strategies (non‑clinical support only) Support consumer's efforts to manage symptoms, using the Illness Management and Recovery Model Assist clients in accessing public benefits, entitlements, and supports, including Medicaid, SSI/SSD, housing, and transportation Facilitate linkage to community resources such as outpatient treatment, vocational services, peer supports, and social services Provide transportation in agency-owned vehicle as needed to access and maintain linkage to community services Provide crisis support and intervention consistent with agency policy, including coordination with screening centers and emergency services when necessary Monitor client progress toward service plan goals and adjust interventions based on clinical and functional needs Maintain timely, accurate, and compliant documentation in electronic health records in accordance with DMHAS and Medicaid requirements Participate in clinical supervision, team meetings, case conferences, and required trainings Uphold ethical standards, client confidentiality, and HIPAA compliance at all timesQualifications:
REQUIRED
Bachelors Degree in a Human Services field from an accredited university OR HS Diploma plus 3 years verifiable work experience in a community-behavioral healthcare setting.REQUIRED
valid NJ drivers license with driving record acceptable to agency insurance carrier.PREFERRED
Minimum one (1) year of experience in community mental health or behavioral health services preferredBenefits:
