Job Description

Overview Pinnacle Support Services, is hiring a Case Manager to oversee not limited to, Supported Living Services (SLS) for individuals with intellectual and developmental disabilities. You'll manage day-to-day service coordination, supervise Direct Support Professionals (DSPs), serve as a liaison between consumers, families, and ensure all documentation meets Title 17 compliance standards.

Program Director Caseload:
8-12 active consumers Responsibilities Conduct intake assessments, home visits, and develop Individualized Service Plans (ISPs) for new SLS consumers Supervise Direct Support Professionals; provide coaching, feedback, and address performance or interpersonal issues Serve as primary point of contact for consumers and families; address concerns and coordinate with Regional Center Service Coordinators Review Daily Service Reports (DSRs), incident reports, and service documentation for Title 17 compliance Monitor consumer progress against ISP goals monthly; prepare quarterly service summaries Facilitate consumer participation in quarterly Individualized Program Plan (IPP) reviews and WRC meetings Coordinate staff schedules, coverage, and emergency response protocols Cover emergency call-offs and uncovered shifts when on-call staff are unavailable; provide direct service as needed to ensure continuity of care and consumer safety High school diploma or GED; Associate's or Bachelor's degree in Human Services or related field preferred Minimum 2 years supervisory or case management experience in developmental disabilities or social services Familiarity with Title 17 regulations, Individual Program Plans (IPPs), and community-based support models Strong written and verbal communication; conflict resolution skills Current CPR and First Aid certification Valid California driver's license and proof of auto insurance DOJ Live Scan fingerprint clearance (we'll arrange) Willingness to work flexible hours and cover shifts as needed for consumer safety and service continuity Required Qualifications High school diploma or GED; Associate's or Bachelor's degree in Human Services or related field preferred Minimum 2 years supervisory or case management experience in developmental disabilities or social services Familiarity with Title 17 regulations, Individual Program Plans (IPPs), and community-based support models Strong written and verbal communication; conflict resolution skills Current CPR and First Aid certification Valid California driver's license and proof of auto insurance DOJ Live Scan fingerprint clearance (we'll arrange) Willingness to work flexible hours and cover shifts as needed for consumer safety and service continuity How to Apply Email your resume and a brief cover letter to jordan@pinnaclesupportservices.net .
