EDUCATION and/or
EXPERIENCE
To qualify a candidate MUST have a
VALID QMHP
Certification and MCR Training Certificate by DBHDS or Region 4 Hub.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Crisis Intervention and crisis response. Interact with Mobile Crisis Dispatch Hub. Coordinate with assigned team members for all dispatches. Ability to report to assigned case within one hour of accepted dispatch. Coordinate assessments with assigned Clinician. Provide updates to Program Manager on case status. Effective de-escalation and stabilization of acute crisis symptoms. Assist in the development and ongoing monitoring and implementation of Individual Service Plan and safety plan. Work with clients to foster the development of new improved coping, problem solving, and communication skills. Case management and care coordination functions of linking, arranging for services and referrals Maintain up-to-date client records, and prepare required/necessary reports in a timely and accurate manner
SUPERVISION
LMHP or a licensed - eligible mental health professional will provide clinical supervision weekly, with individual face-to-face supervision occurring at least once per month.
TRAINING
Staff must be trained certified in: CPR, First Aid, TOVA and become orientated to the company's guidelines, policy and procedures, and take part in staff trainings as they occur.
TRANSPORTATION
Case Managers are required to use their personal vehicles for visits and for engaging in community activities. Therefore at the time of hire, case managers must provide proof of driver's license and proof of automobile insurance.
