Job Description

Position Overview Central Florida Mental Health Services is seeking a compassionate, organized, and motivated Certified Behavioral Health Case Manager (CBHCM) to provide case management services to individuals and families experiencing behavioral health challenges. The ideal candidate will assist clients in accessing community resources, coordinating care, advocating for services, and supporting their overall treatment goals. This position requires a strong commitment to client-centered care, cultural competence, and the ability to work effectively with diverse populations. Candidates must be bilingual in English and Spanish. Minimum Qualifications Bachelor's degree in a behavioral health-related field, including: Psychology Social Work Mental Health Counseling Human Services Behavioral Sciences Sociology Marriage and Family Studies Other related mental health disciplines Certified Behavioral Health Case Manager (CBHCM) credential preferred or eligibility to obtain certification. Fluent in both English and Spanish (written and verbal). Valid Florida driver's license and reliable transportation. Ability to pass background screening requirements. Strong communication, documentation, and organizational skills. Proficiency with electronic documentation systems and Microsoft Office applications. Essential Duties and Responsibilities Conduct assessments to identify client needs, strengths, and barriers to care. Develop and implement individualized service plans. Coordinate and monitor behavioral health, medical, educational, vocational, and social services. Assist clients with accessing community resources, benefits, and support services. Advocate for clients to ensure continuity of care and service accessibility. Maintain timely and accurate documentation in accordance with agency, state, and payer requirements. Collaborate with therapists, psychiatrists, healthcare providers, schools, community agencies, and family members. Monitor client progress and revise service plans as needed. Participate in multidisciplinary team meetings and case reviews. Provide crisis support and referrals when appropriate. Preferred Qualifications Previous experience working with children, adolescents, adults, or families in behavioral health settings. Knowledge of community resources throughout Central Florida. Experience working with Medicaid, managed care organizations, and behavioral health systems. Familiarity with trauma-informed care and recovery-oriented practices. What We Offer Flexible schedule.
