Healthcare Consultant is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness. Healthcare Consultant will effectively manage a caseload that includes supportive and medically complex members. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration. Requirements:
Bachelor's degree in Social Work, Health Science, Healthcare Administration or a related field At least 3 years of case management experience At least 1 year of long-term care experience Bilingual Spanish/English (fluent) Proficiency in Microsoft Office such as Word, Outlook, Excel, and Teams Will travel approximately 50%-75% of the time within the region, seeing Members at home, in assisted living facilities, and nursing homes to complete assessments Reliable transportation and insurance For immediate consideration, email your updated resume, job title, and location to Keyera West Talent Acquisition Specialist kwest@anchorstaffing.com Pay:
