submit resume to tfoster.balancehealth@outlook.com Job Overview The Qualified Mental Health Professional (QMHP)/Case Manager provides Targeted Case Management (TCM) services to individuals with mental health needs. This role is responsible for coordinating care, linking clients to services, advocating for client needs, and ensuring compliance with all applicable regulations, including Medicaid and HIPAA requirements. Duties Conduct comprehensive client assessments and reassessments Coordinate and link clients to medical, social, educational, and community resources Maintain regular contact with clients (face-to-face, phone, or virtual as required) Advocate on behalf of clients to ensure access to appropriate services Collaborate with healthcare providers, therapists, and other service providers Maintain accurate, timely, and compliant documentation Ensure all services meet Targeted Case Management (TCM) and Medicaid guidelines Attend team meetings and training Maintain strict client confidentiality in accordance with HIPAA Documentation Responsibilities Document all client interactions in accordance with company and state standards Maintain case notes that are accurate, timely, detailed, and audit-ready Ensure compliance with billing and service documentation requirements Minimum Qualifications Bachelor's degree in Social Work, Psychology, Counseling, or related field Completed CANS and ANSA's Knowledge of mental health diagnoses and community resources Strong organizational and documentation skills Ability to manage a caseload independently Preferred Qualifications Experience in Targeted Case Management or behavioral health Familiarity with Medicaid guidelines and compliance standards Experience working with diverse or high-need populations Skills and Competencies Strong communication and interpersonal skills Ability to build rapport with clients and families Crisis intervention and problem-solving skills Time management and ability to meet deadlines Ethical decision-making and professionalism Work Environment Combination of office, community-based, and remote work Travel may be required to meet clients in the community Flexible scheduling based on client needs Compliance Requirements Adherence to HIPAA and all confidentiality standards Compliance with federal, state, and company policies Participation in audits and quality assurance reviews
