Full Job Description Description:
The licensed mental health provider at ACG Health is responsible for promoting and restoring patients' health through trauma-informed, patient-centered psychotherapy. The incumbent is expected to meet a weekly productivity level of 25 clinical sessions, inclusive of individual and group therapy services. They will provide care primarily to patients living with HIV, and other comorbidities and to members of the LGBTQ+ community, as assigned and in accordance with departmental needs. Responsibilities:
Greet and acknowledge all patients in accordance with agency standards of professionalism, respect, and culturally responsive care Provide trauma-informed, patient-centered mental health services to individuals living with HIV and co-occurring medical and mental health conditions Conduct mental health screenings, biopsychosocial assessments, and treatment planning within scope of practice and agency standards Provide individual, couples, family, and/or group psychotherapy utilizing evidence-based psychotherapeutic approaches appropriate to patient needs Collaborate with medical providers and interdisciplinary team members to coordinate integrated patient care and support continuity of services Participate in regular case consultations and interdisciplinary meetings regarding patient treatment planning, progress, and referrals Connect patients with appropriate services, community resources, crisis services, and higher levels of care as clinically indicated Provide crisis intervention and support as needed Maintain timely, accurate, and compliant clinical documentation in accordance with agency policy, state and federal regulations, and funding requirements, including intake documentation, psychosocial assessments, treatment plans, progress notes, service logs, releases of information, and discharge summaries Complete and submit all clinical documentation and progress notes within 48 hours of service delivery Other duties as assigned
Requirements:
Master's degree in Psychology, Counseling, Social Work, Marriage and Family Therapy, Art Therapy, or related mental health field Active Pennsylvania licensure (LPC, LCSW, LMFT, or equivalent) Minimum of 1-2 years of experience providing outpatient mental health psychotherapy services Clinical Proficiency in Spanish preferred Experience working with individuals living with HIV, chronic medical conditions, psychosis, severe mental illness, trauma, depression, anxiety, or other co-occurring psychiatric conditions Knowledge of trauma-informed care, culturally responsive practices, and integrated care models Experience facilitating individual and group psychotherapy services Familiarity with community resources, care coordination, and interdisciplinary collaboration within healthcare or social service settings Strong clinical documentation and organizational skills, including timely completion of progress notes and treatment documentation Proficient in using electronic health records (EHR) systems Ability to work effectively with diverse patient populations while demonstrating professionalism, empathy, and strong interpersonal skills Service Orientation
