Full Job Description Job Summary:
Provides a well-focused psychosocial and discharge planning assessment of patient and family to develop a treatment plan and goals, appropriate to the age of the patient.
Duties & Responsibilities:
Service Standards:
Consistently adheres to Butler Hospital service standards Demonstrates behavior, which exemplifies the NASW Code of Ethics to ensure quality of care and promote positive department relations. Under the supervision of a social worker licensed by the state of RI, provides a well-focused psychosocial and discharge planning assessment of patient and family to develop a treatment plan and goals, appropriate to the age of the patient. In collaboration with the team, contributes to defining the need for further psychosocial assessments and services to be performed under the direction of or with the doctor. Implements assigned interventions relative to education and planning treatment goals. Completes appropriate documentation in a timely fashion. Demonstrates commitment to professional growth and maintenance of skills for self and other care planning staff. In collaboration with treatment team, performs utilization review functions and serves as the primary liaison with ERO's. Observes relevant hospital and departmental policies and procedures including risk management, infection control and safety.
Requirements:
Education:
BA/BS/BSW or
MA/MS/MSW
Training and Experience:
Training or experience in a psychiatric facility is helpful but not absolutely necessary. Training and/or experience relevant to the specific population served is necessary for care planners with primary assignments to specialty units or continuing Education/In-service in that area will be required
Special Qualifications:
Interest in and commitment to the programs at Butler Hospital and the Department of Psychiatry and Human Behavior at
Brown University Physical and Mental:
Able to do a moderate amount of walking and tolerate the stress and multiple demands of working in a fast-paced environment dealing with a multidisciplinary staff and difficult patient population.
Schedule:
40/80 Monday - Friday Position will be on D3C/ unit info: This unit will focus on providing specialized treatment for our patients with unique and complex needs. In addition to patients with psychotic and mood disorders, patients treated on this unit will include those with severe behavioral disturbances and patients who are intellectually and developmentally disabled.
Pay Range:
USD $27.99 - USD $37.69 /
