Summary:
Responsible for providing case management svcs to psychiatric patients. Provides patient/family education, coordinates outpatient followup and discharge planning. Maintains communication as indicated with outpatient svcs or CSB for continuity of care. Attends court for commited patients as indicated Assists with insurance certification and reviews.
Requirements:
Minimum Bachelors degree in social services field.
Licenses and Certifications:
NA CPR is required for any RN, LPN, PCT, Patient Transporter or other direct patient care provider. Emergency Department, Intensive Care Units, PACU, Medical Emergency Team and Anesthesia require ACLS. Pediatrics, SICU, Emergency Department and Medical Emergency Team also require
