Job Description

Overview We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care and ensuring that individuals receive the necessary services throughout their treatment journey. This position requires a strong understanding of patient assessment, and discharge planning, along with experience in various healthcare settings. Duties Conduct comprehensive patient assessments to determine individual needs and develop personalized care plans. Coordinate with multidisciplinary teams to facilitate effective patient service and care delivery. Manage cases across various settings, including inpatient, outpatient, and post-acute care environments. Ensure compliance with HIPAA regulations while maintaining accurate medical records and documentation. Provide support for patients dealing with behavioral health issues, addiction counseling, and mental health challenges. Engage in utilization management to optimize resource allocation and improve patient outcomes. Participate in diagnostic evaluations and discharge planning to ensure smooth transitions for patients. Qualifications Case management with severe mental health individuals is required Background in case management and severe mental health is required. Familiarity with social work principles and managed care processes is a plus. Experience in case management, including intake procedures and utilization management. Knowledge of behavioral health practices and mental health counseling techniques is beneficial. Strong communication skills with the ability to advocate for patients effectively. Ability to work collaboratively within a team-oriented environment while managing multiple cases efficiently. Part-time, Contract Pay:
$22.00 - $28.00 per hour Expected hours: 20 - 35 per week
Benefits:
Flexible schedule Application Question(s): Available to accompany clients to ALL appointments. Mainly in the morning time or between the hours of 8am and 4pm.
