Job Summary To Apply for this Job Click Here Clinical Case Manager (CHARTER Program) Salt Lake City, UT Full-Time | Benefits Eligible | Community-Based Mental Health Are you passionate about helping individuals living with severe mental illness achieve stability, recovery, and independence? This non profit mental health provider seeking a dedicated and compassionate Clinical Case Manager to join our Community Housing and Assertive Response for Treatment Enhanced Recovery (CHARTER) team. This is a unique opportunity to work directly with individuals experiencing serious mental illness and co-occurring disorders in a community-based setting. As part of a multidisciplinary team, you will provide a combination of clinical counseling, case management, service coordination, crisis intervention, and recovery-focused support to help residents achieve their treatment and life goals. About the Role The CHARTER Clinical Case Manager provides a full spectrum of clinical and case management services to adults with severe mental illness and co-occurring conditions. Services are delivered in residential settings, participant apartments, and community environments. This position requires a strong clinical foundation, excellent interpersonal skills, and a commitment to trauma-informed, recovery-oriented care. You will collaborate closely with counselors, peer support specialists, medical providers, housing staff, and community partners to ensure residents receive comprehensive, person-centered services. Key Responsibilities Clinical Services Conduct resident assessments and ongoing evaluations. Develop, implement, and monitor individualized service plans. Provide individual counseling and therapeutic interventions within the scope of licensure. Facilitate psychotherapy groups and psychoeducational programming. Teach life-skills classes focused on recovery, independence, and community integration. Utilize motivational interviewing, de-escalation techniques, crisis intervention, and other evidence-based practices. Case Management & Care Coordination Connect participants with community resources including healthcare, employment services, educational opportunities, housing programs, and social services. Coordinate appointments, transportation, and supportive services. Engage residents who may be resistant to treatment using recovery-oriented and trauma-informed approaches. Maintain contact with individuals on program waiting lists and assist with resource navigation. Documentation & Compliance Complete timely and accurate clinical documentation, treatment plans, progress notes, and required reports. Maintain compliance with
HIPAA, CFR
regulations, ethical standards, and agency policies. Utilize the First Step House Electronic Health Record system for documentation, billing, and participant tracking. Maintain productivity standards and documentation timelines. Team Collaboration Work closely with multidisciplinary team members to coordinate care. Participate in case reviews, staff meetings, supervision, and training opportunities. Serve as a positive role model while maintaining professional boundaries and ethical standards. Qualifications Required Master's Degree in Social Work, Clinical Mental Health Counseling, Marriage and Family Therapy, or a related behavioral health field. Active Utah license in one of the following: Licensed Clinical Social Worker (LCSW) Clinical Mental Health Counselor (CMHC) Case management experience working with behavioral health populations. Strong assessment, treatment planning, and documentation skills. Ability to work effectively with individuals experiencing severe mental illness and complex psychosocial challenges. Preferred Experience in substance use disorder treatment. Experience serving adults with serious mental illness. Knowledge of recovery-oriented and trauma-informed care principles. Experience utilizing Motivational Interviewing, crisis intervention, and community-based behavioral health practices.
