Job Description

TriSource Staffing Solutions is seeking a Case Manager to support individuals with intellectual and developmental disabilities (IDD) throughout a community-based behavioral health organization in the Charlottesville area. This role focuses on coordinating services, advocating for individuals, and helping them access community resources that support independence and quality of life. The Case Manager works closely with individuals, families, providers, and community partners to ensure services align with each person's individualized service plan. This is an excellent opportunity for professionals passionate about community integration, advocacy, and person-centered care. Key Responsibilities Conduct assessments of service needs for individuals with intellectual and developmental disabilities Develop and maintain Individualized Service Plans (ISPs) based on identified needs and personal goals Coordinate services across community providers, healthcare professionals, and social service agencies Connect individuals to community resources that support habilitation, independence, and life goals Maintain regular contact with individuals, families, and providers to monitor service delivery and satisfaction Conduct home visits and site visits to assess quality of care and progress toward goals Advocate for individuals to ensure their needs, preferences, and rights are supported Develop and maintain crisis plans and assist with transitions in services when needed Monitor individuals' physical and behavioral health needs, including medications and potential side effects Assist individuals with accessing medical care, public benefits, and community services Maintain accurate documentation and case records in compliance with agency, state, and federal requirements Qualifications Education Bachelor's degree in a Human Services field (such as psychology, sociology, social work, or related field) Experience Minimum 1 year of experience working with individuals with intellectual and/or developmental disabilities Certification Requirements Must meet qualifications for Qualified Intellectual Disabilities Professional (QIDP) under Virginia Department of Behavioral Health and Developmental Services guidelines Additional Requirements Valid Virginia Driver's License with an acceptable driving record Reliable personal transportation for work-related travel Ability to obtain CPR and First Aid certification if not already certified
Pay Rate:
$21-$25/hour depending on experience.
