Case Manager Each One Teach One, LLC - 3.8 Hampton, VA Job Details Full-time | Contract $22 - $26 an hour 6 hours ago Benefits Paid training Qualifications Sociology Clinical case management Driver's License Human Services Social Work Associate's degree First aid and CPR Full Job Description Overview We are seeking a compassionate, organized, and experienced Case Manager/Support Coordinator to join our growing team for a residential group home serving individuals with emotional and behavioral health issues. The Case Manager will coordinate services, monitor individualized support plans, advocate for residents, and ensure compliance with Virginia DBHDS, Medicaid Waiver, and licensing regulations. This position plays a vital role in helping individuals achieve greater independence, stability, safety, and quality of life through person-centered planning and community-based supports. The ideal candidate is detail-oriented, professional, compassionate, and knowledgeable about Virginia emotional and behavioral health resources. Duties Coordinate and monitor individualized service plans (ISPs) and person-centered plans for residents Conduct regular face-to-face visits with residents in accordance with Virginia DBHDS requirements Assess resident needs, strengths, risks, and support goals Collaborate with families, guardians, healthcare providers, therapists, and community agencies Link residents to medical, behavioral health, social, vocational, educational, and community resources Monitor implementation of services and ensure supports align with residents' goals and preferences Maintain accurate documentation, assessments, progress notes, and case records Participate in interdisciplinary team meetings and service planning meetings Advocate for resident rights, independence, and community integration Identify and report concerns related to abuse, neglect, exploitation, health, or safety Assist with crisis intervention and emergency planning when needed Ensure compliance with DBHDS, Medicaid Waiver, HIPAA, and agency regulations Monitor resident progress and revise plans as necessary Support discharge planning and transition services when applicable Qualifications Associate or Bachelor's degree in Human Services, Social Work, Psychology, Sociology, Counseling, Healthcare Administration, or related field preferred Relevant experience in developmental disabilities, behavioral health, case management, residential services, or human services may substitute for degree requirements in some cases Minimum of one (1) year experience working with individuals with mental illness preferred Knowledge of person-centered planning and community resources Strong organizational, communication, and documentation skills Ability to manage multiple cases and priorities effectively Ability to work independently and collaboratively within a team environment Valid driver's license and reliable transportation required CPR & First Aid Certification TOVA training

$22.00 - $26.00 per hour
