Job Description
Our brand-new primary substance abuse treatment center is seeking a compassionate, organized, and patient-focused Case Manager to join our growing team. This position plays a critical role in supporting patients throughout their treatment journey by coordinating aftercare services, discharge planning, referrals, and community resources to help individuals achieve long-term recovery success. The ideal candidate will have experience in behavioral health, substance abuse treatment, social services, or discharge planning and thrive in a fast-paced, team-oriented treatment environment. Position Overview The Case Manager is responsible for assisting patients in obtaining needed services through referral coordination, service linkage, discharge planning, and ongoing support throughout treatment. This role works closely with the Clinical Director, therapists, medical providers, and multidisciplinary treatment team to ensure continuity of care and successful transitions into aftercare services. This is a non-clinical care coordination role and does not provide independent psychotherapy or clinical supervision.

Sunday-Thursday or Tuesday-Saturday Key Responsibilities Coordinate discharge planning and aftercare services for patients throughout treatment Conduct case management needs assessments upon admission Assist patients with referrals related to housing, employment, legal matters, insurance, transportation, financial support, and community resources Collaborate with therapists, medical providers, and the treatment team to support patient care and continuity Ensure all requested referrals and follow-up services are completed prior to discharge Communicate with families, employers, outside providers, and legal representatives in accordance with HIPAA and confidentiality guidelines Support patients through crisis situations and assist with prevention of AMA/ACA discharges Maintain accurate and timely documentation in the electronic medical record Participate in multidisciplinary treatment team meetings Address patient concerns and escalate issues appropriately to leadership Ensure discharge documentation and referral information are transmitted securely and promptly Maintain professional boundaries and uphold a respectful, patient-centered environment Follow all safety, infection control, and emergency procedures Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field preferred Relevant behavioral health or social services experience may be considered in lieu of education Experience in substance abuse treatment, behavioral health, discharge planning, or case management preferred Strong knowledge of community resources and referral coordination Excellent communication, organizational, and interpersonal skills Ability to work collaboratively within a multidisciplinary treatment team Understanding of HIPAA, patient confidentiality, and documentation standards CADC or other addiction-related credential is a plus, but not required Preferred Experience Prior experience working in a detox, residential, PHP, or behavioral health treatment setting Familiarity with aftercare planning and community-based recovery resources Experience supporting individuals with substance use disorders and co-occurring mental health conditions Work Environment This position is performed in a behavioral healthcare environment with regular interaction with individuals experiencing substance use and mental health conditions. Candidates should be comfortable working in a fast-paced treatment setting and responding appropriately to urgent situations when necessary. Pay:
