We are seeking a full-time Mental Health Case Manager to join our Adult Services team. This role offers meaningful, challenging work providing case management services to adults living with Serious Mental Illness (SMI). Responsibilities- Serve as the primary provider of clinical case management services to adult consumers meeting SMI criteria.
- Work closely with team members to support psychiatric stability, quality of life, and access to community resources.
- Encourage appropriate use of medical and inpatient services.
- Provide intake assessments, supportive counseling, outreach, transportation as needed, and community/home-based engagement. Required Education and Experience
- Bachelor's degree in a human services or related field.
- One (1) year of experience providing community-based mental health and case management services to SMI and SUD populations.
- Must be registered as a QMHP with the Virginia Board of Counseling.
- Valid Driver's License with a point balance of -6 or higher. OR
- An equivalent combination of education, training, and experience that meets role requirements.
Required Knowledge, Skills, and Abilities:
- Knowledge of SMI and co-occurring populations, psychosocial rehabilitation, case management practices, documentation, and community service systems.
- Strong written, oral, and interpersonal communication skills.
- Ability to connect clients with crisis and emergency services effectively.
- Sound judgment, initiative, and the ability to work cooperatively with Greater Reach and partner agencies.
- Knowledge of laws and regulations related to case management services.
- Proficiency in basic computer skills and software programs.
