Job Description

We are seeking a full-time Mental Health Case Manager to join our Adult Services team. This role offers meaningful, challenging work providing case management services to adults living with Serious Mental Illness (SMI). Responsibilities Serve as the primary provider of clinical case management services to adult consumers meeting SMI criteria. Work closely with team members to support psychiatric stability, quality of life, and access to community resources. Encourage appropriate use of medical and inpatient services. Provide intake assessments, supportive counseling, outreach, transportation as needed, and community/home-based engagement. Required Education and Experience Bachelor's degree in a human services or related field. One (1) year of experience providing community-based mental health and case management services to SMI and SUD populations. Must be registered as a QMHP with the Virginia Board of Counseling. Valid Driver's License with a point balance of -6 or higher. OR An equivalent combination of education, training, and experience that meets role requirements.
Required Knowledge, Skills, and Abilities:
Knowledge of SMI and co-occurring populations, psychosocial rehabilitation, case management practices, documentation, and community service systems. Strong written, oral, and interpersonal communication skills. Ability to connect clients with crisis and emergency services effectively. Sound judgment, initiative, and the ability to work cooperatively with Greater Reach and partner agencies. Knowledge of laws and regulations related to case management services. Proficiency in basic computer skills and software programs. Additional Requirements Fingerprints, State and FBI criminal background checks, drug testing, and Central Registry clearance are required upon selection. Greater Reach maintains a drugfree workplace. Annual Salary Range Grade SC7 $52,576.00 - $59,148.00 Includes excellent fringe benefits.
