Position Summary In this role, you will respond to behavioral health crises in community settings, provide immediate intervention and stabilization services, coordinate care, and connect individuals to appropriate resources and ongoing support. The ideal candidate is trauma-informed, recovery-oriented, and passionate about helping individuals achieve safety, stability, and positive outcomes. What You'll Do:
Crisis Response & Intervention Provide rapid, on-call crisis response services to individuals experiencing behavioral health emergencies. Deliver services in community-based settings including homes, schools, workplaces, and other natural environments. Utilize effective crisis intervention and de-escalation techniques to support stabilization and resolution. Conduct ongoing safety assessments and monitoring for individuals and others involved in the crisis response process. Help reduce unnecessary emergency room visits, hospitalizations, and law enforcement involvement whenever clinically appropriate. Care Coordination & Community Support Develop, implement, and coordinate individualized service plans based on client needs. Connect individuals to behavioral health resources and supports across the continuum of care. Facilitate referrals, linkage to services, and pre-admission screenings when appropriate. Collaborate with behavioral health providers, community partners, and support systems to ensure continuity of care. Assist with discharge planning and recommendations for step-down services following crisis intervention. Client-Centered Care Deliver recovery-oriented, trauma-informed, culturally responsive services that honor each individual's values, experiences, and preferences. Integrate Zero Suicide and Suicide Safer Care principles into service delivery. Support individuals in maintaining stability within their homes and communities whenever possible. Promote positive outcomes through engagement, advocacy, and compassionate care. Administrative Responsibilities Complete all required documentation, progress notes, reports, timesheets, and assessments accurately and within required timeframes. Participate in supervision, staff meetings, trainings, and ongoing professional development opportunities. Maintain availability to provide services during assigned day, evening, overnight, weekend, and holiday shifts. Skills & Competencies Successful candidates will demonstrate: Strong knowledge of DSM-5 diagnoses Crisis intervention and de-escalation expertise Case management and service plan development experience Excellent verbal and written communication skills Strong interpersonal and relationship-building abilities Knowledge of behavioral health systems and community resources Ability to maintain confidentiality and uphold ethical standards Sound clinical judgment and decision-making skills Required Training & Certifications First Aid/CPR Certification Mobile Crisis Training with Mobile Crisis Certification Restraint Techniques Training Ethics and Human Rights Training Behavioral Intervention Training Minimum Qualifications Bachelor's degree in Health and Human Services or a related field from an accredited college or university, including: Social Work Psychology Counseling Qualified Mental Health Professional - Adult (QMHP-A) and QMHP-C credentials as defined by the Department of Behavioral Health and Developmental Services (DBHDS). Experience providing mental health services and case management to individuals with mental illness preferred. Valid driver's license, reliable transportation, and ability to maintain telecommunications access. Strong understanding of counseling and social work theories, techniques, and methodologies. Excellent written documentation and case conceptualization skills. Ability to successfully pass a criminal history and background check. Why Join Infinite Solutions?
