Current, permanent employees of the Dept of VA; Videos Duties Help Total Rewards of a Allied Health Professional The CBOC Senior Social Worker is responsible for the implemention of program policies and procedures developed by the VAMC and by the Social Work service. Trement is based on coherent empirical and theoretical rionale and is consistent with pient rights. Below are some of the job duties for the position: Conducts psychosocial assessments to determine needs of veterans and family members. Participes in discharge planning with other disciplines and responsible for ensuring th discharge plans are executed in a manner th is timely and approprie. Provides individual, family and group counseling as approprie. Provides consultion/educion to veterans and families on community resources, advance directives and VA benefits. Provides consultion/educion to medical teams and providers on social work practice and VA policy and procedures. Acts as liaison between VA and community resources, to include marketing and public relion duties as needed Provides case management to veterans and families, as needed, throughout the continuum of care. Provides a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as approprie. Addresses issues such as Advance Directives and organ donion in accordance with the standards of their work area. Provides wellness/prevention educion and facilite pient/family support groups as approprie. Supervises the practice of social work students, or be a lead and mentor for less experienced social workers. Coordines trement and communice with the care lines to facilite continuity of care for veterans.
