Case Manager

Key Responsibilities:

• Coordinate with medical and clinical staff.
• Monitor utilization review and management processes to ensure appropriate care levels while adhering to HIPAA regulations and maintaining patient confidentiality.
• Utilize electronic health record (EHR) systems proficiently for documentation, case tracking, and communication.
• Conduct discharge planning activities to prepare patients for safe transitions home or to other care facilities.
• Assess and plan cases, facilitate group sessions, and manage case consultations.
• Plan discharges and aftercare.: Requirements
• Experience working with diverse patient populations, particularly behavioral health cases.
• Experience as a Substance Use Disorder and/or Mental Health Case Manager (required).
• Experience with group session facilitation.
• Familiarity with EMR/EHR systems and compliance standards such as HIPAA is essential.
• Excellent communication skills to effectively collaborate with multidisciplinary teams and advocate for patient needs.
• Ability to navigate complex clinical scenarios and discharge planning.
• Experience in social work or case management is highly desirable.
