Job Purpose:
- A Community Service Provider delivers a range of home- and community-based services, funded by multiple sources, to support clients with co-occurring disorders in developing or maintaining the skills needed for integrated, independent living.
Services include, but are not limited to: medication management, independent daily living skills, physical health monitoring, wellness management, and specialized parenting support. The provider uses a combination of
- teaching, coaching, mentoring, and supervision
- to help clients achieve personalized goals and prevent the need for higher-level or more costly supervised care.

)_ + Deliver independent living skills and supportive services that promote client choice, engagement, and successful community-based living. + Assist the case manager with assessments and contribute to the development and implementation of individualized service and treatment plans. + Document all services accurately and within required timelines to ensure compliance with contract and agency standards. + Maintain regular contact with clients and communicate with external stakeholders to ensure needs are met and plans are followed appropriately. + Teach and support clients in daily living skills, including personal care, household management, safety practices, financial management, medication compliance, and effective use of community resources. + Support clients in accessing and maintaining public benefits, medical services, mental health services, and other professional supports essential for stability and independence. + Promote safety in all environments, exercise sound judgment in community settings, and report all incidents and accidents in accordance with policy. + Other duties as assigned.

Minimum Required Education:
High School Diploma/GED required. Bachelor's or master's degree in social work, psychology, rehabilitation or other related field. A degree is not required but is considered beneficial.
- Minimum Required Experience
- Must have at least six months of experience working with individuals who have mental health needs, physical health conditions, and/or dementia.
- A preferred candidate will have a relevant degree or certificate (such as CNA or CBRF) and experience supporting individuals with mental health needs, physical health conditions, dementia, and/or participating in outcomes-based programs.

Knowledge, Skills, & Abilities:
- Knowledge of the rehabilitative needs of individuals with a wide range of disabilities, including those with co-occurring mental health and substance use disorders.
- Knowledge of community resources, public benefits, and service systems such as SSA, SSI, Medicaid/Title XIX, and local health and social service networks.
- Skill in teaching, coaching, mentoring, and supporting clients in developing independent living skills across home and community-based environments.
- Ability to apply sound judgment and respond safely and appropriately to crises or changing client needs in community-based settings.
- Demonstrated proficiency using a laptop, smartphone applications, and a variety of Microsoft programs, with the ability to learn multiple computer platforms and funder-required systems.
- Ability to communicate effectively, respectfully, and professionally with CFI staff, external case managers and care coordinators, clients, families, and community stakeholders.
- Ability to maintain accurate and timely documentation in compliance with contract, billing, and agency requirements.
- Ability to work independently, manage time effectively, prioritize tasks, and follow structured service and treatment plans.
- Ability to establish rapport, maintain appropriate professional boundaries, and work with individuals from diverse cultural, socioeconomic, and behavioral backgrounds.

Physical Requirements:
- Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
Visual Acuity:
- The worker is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures; documentation; viewing a computer terminal.
