Job Description

Overview Join our dedicated team as a Case Manager specializing in Intellectual and Developmental Disabilities (IDD) Services in WV. Linx and Sycamore are part of caring network of providers. In this vital role, you will coordinate comprehensive care plans, facilitate access to community resources, and advocate for individuals with IDD to promote independence and quality of life. Your energetic approach will ensure seamless service delivery, empowering clients to achieve their personal goals while navigating complex healthcare and social systems. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals with diverse needs within a dynamic, supportive environment. Responsibilities Conduct thorough patient assessments to determine individual needs, strengths, and goals, utilizing your knowledge of physiology, mental health, and behavioral health. Develop and implement personalized care plans that incorporate medical, behavioral, social, and community resources tailored to each client's unique circumstances. Facilitate intake processes and manage utilization review and management activities to optimize patient care pathways while adhering to HIPAA regulations and medical terminology standards. Maintain accurate medical records, document diagnostic evaluations, and oversee patient service documentation with attention to detail and compliance standards. Advocate for clients by connecting them with appropriate resources such behavioral health services, social work support, and managed care programs. Experience Proven background in case management involving complex patient populations with mental health or behavioral health needs. Familiarity with medical terminology, discharge planning procedures, medical records management, and diagnostic evaluation processes. Experience in utilization management or managed care environments that require detailed patient assessment and resource coordination. Knowledge of HIPAA compliance standards related to patient privacy and data security is essential. Strong skills in serving people who may have and IDD and/or mental health diagnosis. Canidate must be able to meet WV IDD Medicaid standards for education.
