ADMINISTRATOR ESSENTIAL FUNCTIONS
This position requires occasionally lifting a maximum of 35 pounds, with frequent lifting and/or carrying of objects weighing up to 10 pounds. This job also requires walking or standing to a significant degree and involves sitting most of the time with a degree of pushing and pulling of the arms. In a workday this employee must stand/walk 1-4 hours; sit-5-8 hours; use hands for repetitive grasping, pushing and pulling, fine manipulation and keyboarding; and be able to bend, twist, and reach frequently and kneel or climb occasionally. As required by HIPAA, all health information for residents and employees to which you are privy, must be kept in strict confidence both during and after your employment.
POSITION SUMMARY
The Pastoral Care & Admissions Coordinator for the End-of-Life Suites is responsible for overseeing all non-clinical operations for a 5-bed licensed SNF/NF unit dedicated to residents receiving end-of-life care and hospice respite services within a Catholic healthcare ministry. This role integrates compassionate admissions coordination with spiritual and pastoral support, ensuring that residents and families experience dignity, peace, and the healing presence of Christ during the final stage of life. The Coordinator serves as the primary liaison for residents, families, hospice partners, clergy, and referral sources, while upholding the mission and values.
MISSION INTEGRATION
Uphold and promote the Catholic identity and mission of the organization. Ensure care and decision-making reflect Catholic moral teaching and respect for the sanctity of life from conception to natural death. Facilitate access to the Sacraments and spiritual support for residents and families. Support an environment of prayer, reverence, and dignity in the End-of-Life Suites. Essential Responsibilities Admissions & Referral Coordination Serve as primary contact for all referrals to the St. Joseph Hospice Suites. Coordinate and complete the full admission process including: Inquiry management Insurance/benefit verification (Medicare, Medicaid, hospice, private pay) Pre-admission screenings Required documentation and consent forms Conduct compassionate tours and family consultations. Collaborate with hospice agencies to coordinate seamless transitions. Maintain census goals for the 5-bed unit. Develop referral relationships with hospitals, parishes, hospice providers, physicians, and community partners. Pastoral Care & Spiritual Support Conduct spiritual assessments upon admission. Provide pastoral presence, prayer, and emotional support to residents and families. Coordinate visits from priests, deacons, Eucharistic ministers, and chaplains. Facilitate reception of the Sacraments (Anointing of the Sick, Eucharist, Reconciliation) when desired. Offer grief support and bereavement follow-up after a resident's passing. Coordinate memorial practices in alignment with Catholic tradition. Family Advocacy & Non-Clinical Oversight Serve as primary family advocate for non-clinical concerns. Facilitate care conferences in collaboration with the interdisciplinary team. Coordinate discharge planning for hospice respite residents returning home. Ensure the End-of-Life Suites maintain a peaceful, reverent, and home-like environment. Operational Oversight (Non-Clinical) Oversee daily non-clinical operations of the 5-bed unit. Ensure rooms are prepared in a welcoming and dignified manner. Coordinate hospitality accommodations for families. Collaborate with nursing leadership to ensure smooth communication. Participate in QAPI initiatives related to end-of-life services. Ensure compliance with SNF/NF regulations related to admissions and documentation.
Qualifications Education:
Bachelor's degree in Theology, Pastoral Ministry, Social Work, Healthcare Administration, or related field preferred. Certification in pastoral care or chaplaincy preferred. Eucharistic Minister required
Experience:
Experience in long-term care, hospice, pastoral ministry, or healthcare admissions preferred. Understanding of Catholic sacramental life and end-of-life ethical directives required. Knowledge of Medicare, Medicaid, and hospice reimbursement preferred.
