About Corewell Health South
  • Lakeland Hospitals The health system comprises three hospitals, an outpatient surgery center, a regional cancer center, rehabilitation centers, two long-term care residences, home care and hospice services, and 34 affiliate physician practice locations.
We provide both inpatient and outpatient medical services throughout Southwest Michigan. Scope of Work Conducts and records spiritual assessments of patients in a manner which is useful to departmental colleagues and members of interdisciplinary healthcare teams. Provides pastoral care, counsel, emotional support and spiritual/religious rituals—as requested and appropriate
  • to patients, families and staff.
Otherwise, the chaplain identifies additional, available resources and assists with referrals. Carefully documents interventions provided. Collaborates closely with departmental colleagues around shared responsibilities, timely referrals, accurate charting and logging, flexibility in helping with unexpected involvements during a shift, making patient hand-offs and follow-ups, taking pager coverage, and offering mutual support. Collaborates with members of the interdisciplinary health care team in identifying holistic approaches to patient care needs, and in facilitating progress towards resolution of these needs or the patient's/family's adjustment to illness or injury. Plans and leads chapel and memorial services, as assigned/authorized, according to a schedule or on special occasions. Provides sacraments in fitting ways and contexts as needs/requests for them emerge. Offers to make appropriate referrals when a particular valid request is outside of the chaplain's ability to provide. Demonstrates the ability to facilitate small groups within the organization or community (e.g., committee leadership, support group guidance and facilitation of patient/family decision making). Provides in-service education for relevant hospital units, nursing/medical students, extended care and hospice personnel, area seminaries and community faith group leaders, Represents department on assigned committees and task-forces. Demonstrates a commitment to ongoing learning and professional development in both intra-and inter-disciplinary areas. Takes initiative in identifying and accessing relevant opportunities for learning and growth within the organization and community. Qualifications Required Master's Degree of divinity or arts in theology with concentration in pastoral care 2 years of relevant experience pastoral care CRT-Chaplain and Fellow
