This position is ideal for someone who is passionate about providing spiritual care, emotional support, and grief services to patients and families during one of life's most meaningful journeys. As a valued member of our hospice team, you will provide individualized spiritual support while coordinating bereavement services that continue beyond the patient's passing. Position Summary The Hospice Chaplain / Bereavement Coordinator provides spiritual counseling, emotional support, grief education, and bereavement services for hospice patients, families, caregivers, and staff. This role collaborates closely with nurses, physicians, social workers, aides, volunteers, and other interdisciplinary team members to ensure holistic end-of-life care that honors each patient's beliefs, values, and preferences. Responsibilities Conduct comprehensive spiritual assessments for hospice patients and families. Develop individualized spiritual care plans based on patient goals, beliefs, and preferences. Provide spiritual counseling, prayer, scripture reading, rituals, sacraments (when appropriate), and emotional support. Respect and support patients of all faiths, cultures, and belief systems. Coordinate and oversee the hospice bereavement program in accordance with Medicare Conditions of Participation. Complete bereavement risk assessments and develop individualized bereavement plans. Provide grief counseling, follow-up calls, memorial services, support groups, and bereavement mailings. Offer crisis intervention and support to families during active dying and at the time of death. Participate in Interdisciplinary Group (IDG) meetings and contribute to the patient's plan of care. Maintain timely and accurate documentation in accordance with hospice, state, and federal regulations. Assist with community outreach, memorial events, volunteer education, and staff support as needed. Qualifications Bachelor's degree in Theology, Divinity, Pastoral Care, Religious Studies, Counseling, or related field preferred. Master of Divinity (MDiv) or equivalent theological education preferred. Previous experience as a hospice chaplain, hospital chaplain, pastor, clergy member, or in spiritual care preferred. Experience providing bereavement counseling or grief support strongly preferred. Knowledge of hospice philosophy and end-of-life care principles. Excellent interpersonal, communication, counseling, and active listening skills. Ability to provide compassionate, nonjudgmental spiritual care to individuals of diverse faiths and backgrounds.
