Responsibilities for this role include:

Conducting initial and follow-up Chaplain visits both in-person and via phone Conducting initial and follow-up bereavement assessments both in-person and via phone Assisting in all aspects of bereavement program from initial admission through 13-months post-passing Assisting in coordinating and hosting annual Celebration of Life ceremony Fulfilling ad hoc patient care concerns by coordinating across both internal and external stakeholders Voicing your opinion is key!
