Divinity Full Job Description Major Responsibilities:
Provides initial assessment of existing spiritual resources and preferences of patients. Provides spiritual care and emotional support to patients and families. In conformance with the patient's faith and expressed desire, pastoral care services of counseling, sacraments, and support will be available. Responsible for development of a spiritual plan of care reflecting problems, interventions, and outcome goals. Implements the plan of care in conjunction with the hospice interdisciplinary team and according to hospice specific protocols and measures. Serves as liaison for the hospice patient and their local faith community. Facilitates conversations specific to end-of-life planning including goals of care, funeral arrangements, and legacy work. Collaborates with hospice interdisciplinary team with referrals of patients who are in crisis situations. Participates in the QA/CQI process for Advocate Hospice. Responsible for achievement of agency, department and personal goals and objectives. Licensure, Registration and/or
Certification Required:
A valid driver's license issued by the Division of Motor Vehicles.
Education/Experience Required:
Master's Degree in Theology, or Master's Degree in Divinity, or Master's Degree in Religious Studies. At least Two (2) units of CPE, accredited by ACPE; NACC units may be considered. 5 units preferred. In good standing with continued endorsement by the appropriate authority of his/her church or religious institute
Knowledge, Skills & Abilities Required:
A strong background in functional theology and the ability to apply this in a pastoral manner to individuals and families. Excellent communication skills with knowledge of counseling techniques. Ability to identify client's needs with consideration to cultural, ethnic, religious, economic, and other diverse backgrounds. Experience and compassion in ministering to people who are dying and their grieving families.
