POSITION SUMMARY
On-call as needed. Responsible for providing rehabilitative stabilization services for adults with severe mental illness or dually diagnosed. Teaches in-vivo independent, social, and daily living skills taking into consideration the participant's learning style, strengths and needs. Assists participants in the acquisition and practice of daily living skills. Monitors participants' symptoms and medication utilization regularly. Maintains an organized, clean, and safe home environment. Provides crisis management services in consultation with program supervisor. Documents all services using program protocols. Requirements High School Diploma or equivalency (GED). Two years of experience in the human services field preferred. Experience working with multi-ethnic populations. Flexibility to work on weekends, evenings, holidays during emergencies or staff shortage.
