Job Description

Job Summary:
Resident Service Assistant at Henderson Health Care Services Henderson Health Care Services (HHCS) is looking for a full-time Resident Service Assistant at Henderson's Long Term Care Facility, Legacy Square. The Resident Services Assistant is responsible for coordinating resident appointments, maintaining up-to-date files and records, and assisting the Director of Nursing with daily and monthly administrative tasks. This role supports both clinical and social services functions, helping ensure residents receive timely care and supportive services while maintaining accurate documentation.
Key Responsibilities:
Ensure resident safety during transportation to and from medical appointments. Prepare and organize necessary paperwork for resident appointments and lab work (e.g., lab tracking, permissions for lab draws). Track and manage 30-, 60-, and 90-day follow-up appointments; send reminders to residents and families. Answer phones professionally and transfer calls as needed. Maintain organized, up-to-date resident medical records. Assist with admission and discharge paperwork, including file preparation and documentation. Support residents during the admission and adjustment process. Complete admission/discharge logs and maintain daily census documentation. Provide one-on-one interactions and support to residents. Complete periodic assessments of residents' mood and mental status; chart progress notes for social service interventions when needed. Contact resident representatives or family members regarding personal needs or concerns. Assist in collecting and recording residents' social histories. Help coordinate and support Resident Family Council Advisory Committee meetings. Maintain accurate records and reporting of resident trust accounts. Monitor and restock office supplies to ensure availability. Greet and assist visitors, residents, and families in a professional and courteous manner.
Qualifications:
To perform job successfully, an individual must be able to multi-task, pay attention to detail, be flexible and dependable. Must have basic computer knowledge, including but not limited to, Word, Excel, and E-mail. Have good presentation/public greeting skills.
Education:
40 hour Activity Director Course; Social Service Designee Course (can be obtained on the job).
