Job Summary:

Creative Health Services is looking for Full and Part-Time and Per Diem shifts including overnight with flexible hours for all shifts.

What You Will Do:

Teach/assist residents with daily living skills in a transitional residential setting. Monitor residents taking medications. Complete chart documentation in electronic health record. Supervise resident activities in the community.

What You Will Need:

A Degree in the Human Services field preferred but will consider candidates with experience in the Mental Health Field. Valid PA Driver's License with a good driving report. PA Criminal Background Check, Child Abuse Clearance, FBI Fingerprint Clearance, Act 31 Training, and NSOR (National Sex Offender Registry) Verification are required.
