Position Summary:
The Housing Transition Assistant (HTA) supports individuals with Intellectual and Developmental Disabilities (IDD) in securing and maintaining safe, stable, and independent housing. This role provides hands-on assistance with housing-related activities, documentation, and community-based support under the direction of a Housing Coordinator or Team Lead.
Position Scope:
The HTA operates in a hybrid, community-based setting across Pennsylvania, supporting Housing Transition and Tenancy Sustaining Services (HTTS). The role focuses on executing day-to-day tasks while supporting service delivery, compliance, and individual outcomes.
Key Responsibilities:
Support individuals with housing search, applications, and transition processes, including locating housing options (such as apartments, shared housing, and vouchers), gathering and organizing required documentation, completing applications, following up with landlords, coordinating and attending housing tours, and assisting with move-in logistics such as utilities and furnishings. Assist with tenancy-sustaining activities and reinforce independent living skills, including budgeting, housekeeping, and lease comprehension, while providing direct, community-based support aligned with Individual Support Plans (ISP). Ensure accurate and timely documentation of services in systems such as C2S, track progress toward housing goals, and maintain compliance with program requirements. Maintain consistent communication with internal and external stakeholders, support audit preparation and documentation collection, assist with home accessibility coordination when needed, and participate in team meetings, case discussions, and general administrative and coordination activities.
Regulatory Frameworks:
Pennsylvania ODP Waiver requirements Housing Transition and Tenancy Sustaining Services (HTTS) guidelines Applicable federal housing programs (e.g., HUD/Section 8) Agency policies and compliance standards
Confidentiality:
Maintains strict confidentiality of all individual, medical, and housing-related information in compliance with HIPAA and organizational policies.
Documentation Requirements:
Complete accurate and timely service documentation in C2S or related systems Maintain records for housing applications and services delivered Ensure documentation meets audit and authorization requirements
Certifications:
CPR/First Aid (preferred or required upon hire per company policy) Any required state-mandated direct care or IDD-related training
Education:
High School Diploma or GED required
Experience:
Experience in IDD, behavioral health, housing services, or human services preferred
Technical Skills:
Basic computer skills (documentation systems, email, MS Office) Ability to learn internal systems such as C2S Organizational and time management skills
Languages:
English is required. Additional languages are a plus.
Driver License:
Valid driver's license and reliable transportation required.
Travel Requirement:
Frequent local travel within the assigned Pennsylvania region and community-based work required.
