Our team is dedicated to delivering tailored services that promote independence, dignity, and overall well-being. We are seeking a compassionate and skilled individual to join our social services department and contribute to our mission of delivering exceptional care.
Responsibilities:
Assist the social worker in conducting assessments, developing care plans, and coordinating services to meet patients' and residents' social, emotional, and psychological needs Provide emotional support and counseling to patients, residents, and their families Facilitate communication between patients, residents, and healthcare providers to ensure seamless care transitions Coordinate community resources and services to enhance patients' and residents' quality of life Maintain accurate and timely records of patient and resident interactions, assessments, and care plans Collaborate with interdisciplinary teams to develop and implement individualized care plans About the
